Sliding Into Safety: Protecting Young Baseball Players from Overuse Injuries

Baseball may look like a low-contact sport compared to football or soccer, but for young pitchers and catchers, it carries one of the highest overuse injury risks in all of youth athletics. As Little League and travel ball rosters fill back up this season, here's what parents need to know to keep young arms healthy all year long.


The Injury Parents Don't See Coming


When people picture a baseball injury, they usually think of a foul ball or a collision at home plate. In reality, the sport's biggest threat to kids is quieter and slower-building: repetitive stress on the shoulder and elbow from throwing.


Research on youth pitchers has consistently identified pitch volume as the single strongest predictor of injury; the more pitches thrown per game, per week, and per season, the greater the risk of shoulder and elbow damage. Traumatic injuries, like an avulsion fracture at the elbow, happen far less often than the slow accumulation of overuse damage that can lead to conditions like "Little League shoulder" or elbow ligament injuries requiring Tommy John-style reconstruction later in a young athlete's career.


What makes this especially concerning is a knowledge gap. Studies of youth coaches have found that while a majority say they track pitch counts, a large share aren't tracking them against the age-based guidelines established by USA Baseball's Medical & Safety Advisory Committee, and only a small fraction of coaches surveyed could correctly identify overuse risk factors. Surveys of the young athletes themselves show similar gaps; many players don't know the safe-throwing guidelines meant to protect them.


Prevention Tips for Parents


1. Know (and track) the pitch count guidelines.

Pitch Smart, a joint initiative from Major League Baseball and USA Baseball, publishes age-specific daily pitch limits and required rest days between outings. These numbers exist because research shows exceeding them significantly raises injury risk even at the youngest levels.


2. Push for rest, not just recovery.

Guidelines call for at least a few months completely off from throwing each year. If your child plays on a school team, a travel team, and attends private lessons or showcases simultaneously, their "innings" add up fast even if no single coach realizes it.


3. Delay breaking pitches.

Curveballs and sliders thrown before the growth plates mature are linked to increased injury risk. Most safety guidelines recommend holding off on breaking pitches until the mid-teens.


4. Take arm soreness seriously

Historically, "pitching through pain" was treated as toughness. Sports medicine providers now view shoulder or elbow soreness as an early warning sign, not something to push through. Encourage your child to report discomfort immediately, and don't let a "big game" be the reason it gets ignored.


5. Build the whole kinetic chain, not just the arm.

Throwing mechanics rely on the hips, core, and shoulder blade working together. A movement or motion analysis can reveal compensations like restricted hip rotation or scapular instability years before they show up as pain.


When to Bring in a Sports Medicine Provider


If your young pitcher or catcher reports recurring soreness, has noticeably reduced velocity or accuracy, or complains of pain that lingers more than a day or two after playing, it's worth a professional movement evaluation rather than waiting to see if it resolves on its own.
