Patient Responsibilities
As a patient of Patel Peak Performance, you agree to:

Provide accurate and complete health history information at intake and update us promptly if your health status changes

Follow your care plan as recommended, and communicate openly with your provider if you have concerns or experience any adverse reactions

Arrive on time for scheduled appointments and provide adequate notice if you need to cancel or reschedule (see our Cancellation Policy below)

Appointment & Cancellation Policy
We reserve appointment times specifically for you. To respect both your time and the time of other patients:

We ask for a minimum of 24 hours' notice for any cancellation or reschedule request

Missed appointments or late cancellations (less than 24 hours' notice) may be subject to a cancellation fee

Repeated no-shows may result in a requirement to prepay for future appointments
To cancel or reschedule, please contact us by phone during office hours
Payment
Payment is due at the time of service unless prior arrangements have been made. For care packages and promotional offers, please refer to our Refund & Promotions Policy.
Good Faith Estimate (No Surprises Act)
Under the No Surprises Act, you have the right to receive a Good Faith Estimate of the expected cost of your care before your appointment if you are uninsured or choosing not to use your insurance. If you would like a Good Faith Estimate prior to your visit, please contact our office and we will provide one promptly.
Informed Consent
Before beginning care, all patients are required to review and sign an informed consent form. This form explains the nature of chiropractic treatment, potential risks and benefits, and your right to ask questions or withdraw from treatment at any time. Chiropractic care is generally very safe; however, as with any healthcare service, there are inherent risks. Your provider will discuss these with you thoroughly before beginning treatment.
Minor Patients
Patients under the age of 18 must have a parent or legal guardian present at their first visit to sign all required consent forms. A parent or guardian must authorize all treatment for minors
